Práce se zaměřuje na tradiční symptomy přenosu a na to, zda se ve zkoumaných rodinách objevily, nebo ne a proč, na identitu přeživších a jejich děti a na to, jaký je rozdíl v paměti mezi první a druhou generací přeživších. 2024 - Czech Souhrn Incidence a mortalita nádorů gastrointestinálního traktu jsou na úrovni, která představuje závažný problém. Tyto nádory mají nespecifické klinické příznaky, a proto často dochází k pozdní diagnóze. V posledních letech byl zaznamenán výrazný pokrok v oblasti vyhledávání a sledování molekulárních ukazatelů nádorových změn v diagnostice, prognóze a léčbě těchto onemocnění. Klinická onkologie se však stále potýká s nedostatkem takovýchto biomarkerů. Cílem této práce bylo najít nové biomarkery pro nádory gastrointestinálního traktu, které by korelovaly zejména s prognostickými ukazateli a prediktory léčebné odpovědi či chemorezistence. Dizertační práce je založena na 5 studiích věnujících se duktálnímu adenokarcinomu pankreatu, kolorektálnímu karcinomu a hepatocelulárnímu karcinomu s použitím imunohistochemických a molekulárních metod. Všechny studie byly provedeny na archivních parafínových blocích. K hlavním výsledkům patří imunohistochemicky prokázaná proteinová exprese transportérů MRP2, SLC22A3 a SUR1/ABCC8 a její významná souvislost s prognózou adenokarcinomu pankreatu. Bylo zjištěno, že pacienti bez exprese proteinu SLC22A3 v apikální membráně mají signifikantně kratší přežití bez nemoci i celkové přežití. U pacientů s pozitivní membránovou expresí MRP2 i SLC22A3 jsme nalezli delší přežití bez... The incidence and mortality of gastrointestinal cancers are at a level that represents a serious problem. These tumours have non-specific clinical symptoms, which often results in late diagnosis. In recent years, there have been significant advances in the identification and monitoring of molecular markers of tumour changes in the diagnosis, prognosis and treatment of these diseases. However, clinical oncology still faces a shortage of such biomarkers. The aim of this study was to find new biomarkers that correlate especially with prognostic indicators and predictors of treatment response or chemoresistance in gastrointestinal tract cancers. This thesis is based on 5 studies addressing ductal adenocarcinoma of the pancreas, colorectal cancer and hepatocellular carcinoma using IHC and molecular methods. All studies were performed using archival formalin-fixed paraffin-embedded samples. The main results include immunohistochemical evidence of protein expression of MRP2, SLC22A3 and SUR1/ABCC8 transporters and its significant association with the prognosis of pancreatic adenocarcinoma. 2024 - Czech Právní postavení osob využívajících asistenci psů se speciálním výcvikem Abstrakt Diplomová práce se zabývá problematikou právního postavení osob využívajících asistenci psů se speciálním výcvikem. V českém právním řádu nadále chybí definice psa se speciálním výcvikem a jeho jednotlivých druhů. Požadavky na výcvik, dovednosti a označení speciálně vycvičených psů nejsou českým právem upraveny vůbec. Navzdory finanční a časové náročnosti speciálního výcviku je v současné době poskytován pouze příspěvek na pořízení zvláštní pomůcky v podobě vodicího psa. Právní úprava přístupových práv osob v doprovodu psa se speciálním výcvikem je roztříštěná do předpisů různé právní síly, úprava některých oblastí je zcela opomenuta. Prostředky obrany při neoprávněném odepření přístupu osoby v doprovodu psa se speciálním výcvikem v praxi narážejí na zásadní limity. Cílem práce je podat přehled stávající právní úpravy stěžejních aspektů právního postavení osob využívajících asistenci psů se speciálním výcvikem. Na základě toho jsou autorkou navrženy změny, které mohou přispět k jejich plnohodnotné integraci do společnosti na rovnoprávném základě s ostatními. Práce je rozdělena do čtyř částí. The Czech legal system still lacks a definition of a dog with special training and its various types. The requirements for training, skills and marking of specially trained dogs are not regulated by Czech law at all. Despite the financial and time demands of special training, currently only an allowance is provided for the purchase of special equipment in the form of a guide dog. The legal regulation of the access rights of persons accompanied by a specially trained dog is fragmented into regulations of different legal force, the regulation of some areas is completely absent. In practice, the means of defense in case of unjustified denial of access to a person accompanied by a dog with special training encounter significant limits. The aim of the thesis is to give an overview of the current legal regulation of major aspects of the legal status of persons using assistance of dogs with special training. 2024 - Czech Dobrovolné nástroje ochrany životního prostředí Abstrakt Tématem této diplomové práce jsou dobrovolné nástroje ochrany životního prostředí. Hlavním cílem je vytvořit ucelený a komplexní přehled nejvýznamnějších dobrovolných nástrojů, charakterizovat je, demonstrovat způsoby jejich využití v praxi a také analyzovat jejich právní regulaci na národní, evropské anebo mezinárodní úrovni. Pro adekvátní představení tématu je úvodní část této práce věnována obecně nástrojům ochrany životního prostředí. Druhá část se detailněji zabývá již jen skupinou dobrovolných nástrojů, přičemž popisuje také historický kontext jejich vzniku v rámci environmentální politiky. Další části této práce se zaměřují na konkrétní dobrovolné nástroje, jež byly vybrány jako objekt bližšího zkoumání. Tyto nástroje jsou seřazeny podle jejich vyhodnocené právní a praktické relevance. 2024 - Czech Tato diplomová práce se věnuje komplexní analýze preventivní restrukturalizace v kontextu českého insolvenčního práva s důrazem na její praktickou aplikovatelnost a přínosy pro obchodní korporace čelící finančním obtížím. Zvláštní pozornost je věnována směrnici Evropské unie o preventivní restrukturalizaci, jejímž cílem je harmonizovat přístupy k řešení finančních problémů v členských státech a poskytnout efektivní nástroje pro předcházení úpadku podniků. Práce zkoumá legislativní rámec preventivní restrukturalizace, analyzuje její implementaci v českém právním řádu a srovnává ji s přístupy v jiných zemích. Na základě teoretického zkoumání a analýzy empirických dat práce hodnotí, zda preventivní restrukturalizace představuje vhodný nástroj pro sanaci a zda dokáže v praxi naplnit očekávání spojená s jejím zavedením. Special attention is given to the European Union Directive on preventive restructuring, aimed at harmonizing approaches to solution of financial difficulties across member states and providing effective tools to prevent business failure. The thesis examines the legislative framework of preventive restructuring, analyzes its implementation into Czech legal system, and compares it with approaches in other countries. Through theoretical examination and empirical data analysis, this thesis assesses whether preventive restructuring can be a suitable tool for financial rehabilitation and whether it can fulfil the expectations associated with its introduction. Key findings indicate that although preventive restructuring represents a debtor-friendly and less procedurally demanding approach, its real impact on the business environment in the Czech Republic will be limited. 2024 - English This research aimed to elucidate the role and importance of the millipede gut microbiome in cellulose digestion by using inhibitors to disrupt potential host-symbiosis and assessing their effects on millipede digestion and overall health. It involved the first comprehensive profiling of microbial communities within the hindgut and faeces of two distinct millipede species: Epibolus pulchripes, a tropical species found on the East African coast, and Glomeris connexa, a temperate species native to Central Europe. Although both species share a similar detritivorous lifestyle, they differ in size and gut redox conditions, with G. connexa being smaller (10-17 mm) than E. pulchripes (130-160 mm). The study also revealed the potential of the hindgut bacterial community in breaking down complex polysaccharides and recycling nutrients. It described the active bacterial community vital for certain processes and the extent of the millipedes' dependence on them. Additionally, the research provided a comprehensive investigation of viral communities in the hindguts of the two millipedes and their role in enhancing metabolism and modulating microbial composition. Furthermore, it introduced a new perspective that millipedes primarily ingest litter to gain access to microbial biomass (primarily fungal), which they and their gut microbiota consume. Cílem této práce je zejména analyzovat právní úpravu oprávnění a povolení stavět, a to s ohledem na rekodifikaci stavebního práva. Vzhledem k tomu, že se jedná o velice aktuální téma, je proto možné v důsledku vývoje právní úpravy přinést nový vhled do této oblasti. Práce je členěna do čtyřech samostatných logicky navazujících částí. Nejprve jsou představeny základní pojmy, které jsou pro téma práce relevantní. Jedná se zejména o pojmy pozemek a stavba, které jsou vyjádřeny v kontextu různé právní úpravy. Jedná se většinou o vymezení podle občanského zákoníku, katastrálního zákona a stavebního zákona. Druhá část se zabývá právním vymezením soukromoprávního oprávnění stavět. V této části jsou zejména vymezeny soukromoprávní tituly a je zde snaha o posouzení vhodnosti pro realizování záměru. Řešena je také otázka vlastnického práva, jakožto opravňujícího titulu ke stavbě. Zmíněno je nabývání vlastnického práva i jeho možné omezení. Na problematiku oprávnění stavět navazuje další část, která je již veřejnoprávní a věnuje se otázce povolení stavět. The diploma thesis entitled Authorization and building permit deals with two basic approaches, authorization and permission to build. The aim of this work is to analyze the legal regulation of permits and building permits, with regard to the recodification of construction law. Given that this is a very current topic, it is therefore possible to bring new insight into this area as a result of the development of legislation. The work is divided into four separated logically connected parts. First, basic concepts relevant to the topic of the work are introduced. These are mainly the concepts of land and building, which are expressed in the context of various legal regulations. These are mostly delineations according to the Civil Code, the Cadastral Act and the Construction Act. The second part deals with the legal definition of the private right to build. In this part, private law titles are defined in particular and there is an effort to assess suitability for the implementation of the plan. The question of ownership is also addressed here, as a title entitling to construction. The acquisition of ownership rights and it is possible limitations are mentioned here. The use of TIME management in nursing practice

2024 - Czech The bachelor thesis deals with the use of the TIME management concept in the practice of nursing. The theoretical part presents knowledge related to wound management, with the primary focus on the concept of TIME management and its use in nursing. In the empirical part, the first objective was to compare the knowledge of the TIME management concept in terms of the education of the nursing staff. The study's second objective was to find out the kind of experience nurses have in TIME management. The third and last objective was to map whether, an000d to what extent, TIME management is being used in selected wards. To meet the objectives, three hypotheses were formulated. The objectives were achieved through quantitative research using a non-standardized questionnaire. The research population included general nurses working in selected wards in the Klatovy hospital. The total number of respondents was 140. The results obtained verified that respondents with higher education more often declared knowledge of the rating scales (p = 0.006), reflecting the importance of educational background when preparing to provide effective care to non-healing wounds. When assessing the level of experience with TIME management, there were no significant changes in wound parameter scores, which related to respondents' length of experience. It was also found that the use of therapeutic methods in TIME management varied by type and specialization of the ward, which is primarily related to the frequency of the occurrence of non-healing wounds in a given workplace. Bakalářská práce se zabývá využitím konceptu TIME managementu v ošetřovatelské praxi. Teoretická část přináší poznatky týkající se managementu ran, přičemž největší pozornost byla kladena primárně na koncept TIME managementu a jeho využívání v ošetřovatelské praxi. V empirické části bylo prvním cílem srovnat znalosti konceptu TIME managementu z pohledu vzdělání ošetřovatelského personálu. Druhým cílem práce bylo zjistit, jakými zkušenostmi v oblasti TIME managementu sestry disponují. Posledním, třetím cílem bylo zmapovat zda, a jakém rozsahu se TIME management na vybraných odděleních využívá. Ke splnění cílů byly formulovány tři hypotézy. Stanovených cílů bylo dosaženo formou kvantitativního výzkumu pomocí nestandardizovaného dotazníku. Výzkumný soubor zahrnoval všeobecné sestry pracující na vybraných odděleních v Klatovské nemocnici a.s. Celkový počet respondentů činil 140. Získané výsledky nám ověřili, že respondenti s vyšším vzděláním častěji deklarovali znalosti hodnotících škál (p = 0,006), což odráží význam vzdělávacího pozadí v přípravě na efektivní péči o nehojící se rány. Při hodnocení míry zkušeností s TIME managementem se neprokázaly výrazné změny v hodnocení parametrů rány, které by souvisely s délkou praxe respondentů. Dále bylo zjištěno, že využití terapeutických metod v TIME managementu se liší podle typu a zaměření pracoviště. Což primárně souvisí s četností výskytu nehojících se ran na daném pracovišti. Birth injuries and their impact on women

2024 - Czech The bachelor thesis is focused on the topic of birth injuries and their impact on women. The thesis includes a theoretical and practical part. The theoretical part of the bachelor thesis describes the types of birth injuries, the reasons for their occurrence and preventive methods. Attention is also given to the diagnosis and treatment of birth injuries and the care of a midwife for birth injuries during the six-week postpartum period. At the end of the theoretical part are summarized the most common impacts on women related to birth injuries. For the practical part of the bachelor thesis were established three objectives and four hypotheses. The first objective was to determine the most common problems women face in relation to third and fourth degree perineal tears. For this objective were determined two hypotheses. H1: Women with third and fourth degreee perineal tears have problems in their sex life. H2: More than 75% of women with third and fourth degree perineal tears experience unwanted gas leakage. The second objective of the bachelor thesis was to map out the role of a midwife in caring for birth injuries. One hypothesis was established for this objective: H3: The role of a midwife in caring for birth injuries involves educating women in this area. The purpose of the third objective was to determine women's awareness in caring for their birth injuries. For this objective was chosen one hypothesis: H4: Women obtain information regarding the care of birth injuries more from midwives than from doctor. The goals were achieved through quantitative research using online questionnaires. The questionnaire contained 18 questions: six closed questions, eleven semi-closed questions where multiple answers could be selected, including an "other" option, and one open-ended question. Statistical processing of all questionnaires was done in Microsoft Excel. The research sample consisted of 620 women who gave birth between March 2019 and January 2024, and who had third and fourth degree perineal tears or an episiotomy during childbirth. Out of the total of 620 respondents, birth injuries had an impact on 446 respondents, while 174 respondents did not experience any problems related to birth injuries. The research revealed that birth injuries (episiotomy and third and fourth degree perineal tears) had an impact on more than 70% of respondents. It was also found that almost 35% of respondents were not educated on the care of birth injuries during hospitalization. Out of the four hypotheses, hypotheses H1, H3 and H4 were confirmed, while hypothesis H2 was not confirmed. Bakalářská práce se věnuje tématu porodní poranění a jejich dopad na ženy. Práce zahrnuje teoretickou a praktickou část. V teoretické části bakalářské práce jsou popsány typy porodního poranění, příčiny jejich vzniku a preventivní metody. Pozornost je dále věnována diagnostice a ošetření porodního poranění a péči porodní asistentky o porodní poranění v období šestinedělí . V závěru teoretické části jsou shrnuty nejčastější dopady na ženy v souvislosti s porodním poraněním. Pro praktickou část bakalářské práce byly stanoveny tři cíle a čtyři hypotézy. Prvním cílem bylo zjistit, s jakými problémy se ženy v souvislosti s rupturami perinea III. a IV. stupně potýkají nejčastěji. K danému cíli byly určeny dvě hypotézy. H1: Ženy s rupturou perinea III. a IV. stupně se potýkají s problémy v sexuálním životě. H2: U více než 75% žen s rupturou perinea III. a IV. stupně se vyskytuje nechtěný únik plynů. Druhým cílem bakalářské práce bylo zmapovat roli porodní asistentky v péči o porodní poranění. Zde byla stanovena právě jedna hypotéza, a to H3: Role porodní asistentky v péči o porodní poranění spočívá v edukaci žen v této oblasti. Účelem posledního, třetího cíle bylo zjistit informovanost žen v péči o jejich porodní poranění. K tomuto cíli byla zvolena také jedna hypotéza. H4: Informace týkající se péče o porodní poranění získávají ženy spíše od porodních asistentek než od lékařů. Cílů bylo dosaženo prostřednictvím kvantitativního výzkumného šetření pomocí vlastních online dotazníků. Dotazník obsahoval 18 otázek. Šest otázek bylo uzavřených, jedenáct otázek polouzavřených, u nichž bylo možné označit více odpovědí, včetně možnosti "jiné" a jedna otázka otevřená. Všechny dotazníky byly statisticky zpracovány v programu Microsoft Excel. Výzkumný soubor tvořilo 620 žen, které rodily v období od března 2019 do ledna 2024 a u nichž došlo při porodu k ruptuře perinea III. a IV. stupně nebo byla provedena epiziotomie. Z celkového počtu 620 respondentek mělo porodní poranění dopad na 446 respondentek a u 174 respondentek se žádné problémy v souvislosti s porodním poraněním nevyskytly. Z výzkumného šetření vyplynulo, že má porodní poranění (epiziotomie a ruptura perinea III. a IV. stupně) dopad na více než 70% respondentek. Dále bylo zjištěno, že téměř 35% respondentek nebylo během hospitalizace edukováno v problematice péče o porodní poranění. Z celkového počtu čtyř hypotéz se potvrdily hypotézy H1, H3 a H4. Hypotéza H2 se nepotvrdila.
